About the
Neighborhood Health Care Network
The Neighborhood Health Care Network is a
management services organization whose mission is "to strengthen
the community clinics through integrated activities so that together
we can improve
the health of underserved communities." Since its incorporation
in 1995, the Network has built a high-quality, cost-efficient
infrastructure
to support the Minneapolis - St. Paul metropolitan areas' primary
and preventive health care safety net. By providing collaborative
management
services,
the Neighborhood Health Care Network maximizes cost-efficiencies
for its member clinics, works to improve clinical and business
functions, and helps to develop and market a comprehensive health
care system
that is competitive in the marketplace.
In 2003, the Network was one of only nine community
health center associations nationally to be awarded funding under
the Federal Bureau of Primary Health Care’s new multi-year
Integrated Services Development Initiative (ISDI) "operating" grant. This designation means that the
Network has achieved a degree of service integration and delivery
that the Bureau considers mature and worthy of operational funding
on a sustained basis.
In addition, the Network manages
a statewide phone service known as 489-CARE that provides health
care information and referral to sliding fee scale clinics. Last
year, these outreach programs provided referrals and information
to over 5000 people and helped over 2000 successfully enroll in
public health insurance programs. Locally, the Network has distinguished
itself as an effective conduit to hard-to-reach populations and
a source of critical health data for and about medically underserved
populations.
